If you are changing the name or the data on your passport, submit original certified documents justifying the change you are requesting. For example, if you want to change it due to marriage, submit an original Certified Copy of a Marriage Certificate. If you are changing vital information such as your date of birth or gender, submit original Certified Copies justifying this change, such as a previously issued passport, a Certified Copy of your Birth record, or a court order.

Note: There is no real position the government takes on what kind of information they expect to have reported on this form, aside from answering the questions clearly, honestly, and accurately.
Often, people would say, “well if I knew where my passport was, it wouldn’t be lost”. It is a good point, however, what they are trying to determine here is if there has been any inappropriate activity such as fraud or other criminal behavior or just negligence on the part of the passport holder or parent, or mor eimportantly wether it can be located.
So, it is best to just explain as much information as you possibly can to help them to determine if you might be able to find it, or if it is lost, is it in the hands of the wrong person.

Eligibility for This Passport Application Method
To use this method of obtaining a U.S. passport, the following must be true:
It does not matter whether the child has had a passport before. As long as the applicant is under 16 now, this process applies.
If the child already has a passport, it must be submitted with the new application. However, you must also include a certified copy of a state or county-issued long-form birth certificate. This document is required because a minor’s passport does not list the parents’ names, and the birth certificate is used to verify parental information.

Note: It is not commonly known that the U.S. State Department issues to qualified applicants 2nd or additional U.S. passports. They are legally issued under specific circumstances by the Federal government.
Normally, they are issued for the following two reasons:
a. Applicant is traveling to countries having known diplomatic conflict with each other, such as Israel and Saudi Arabia. If you have an Israeli embarkation stamp in your current passport, you cannot obtain or apply for a Saudi Arabian visa.
b. Applicant is traveling abroad so frequently and to countries that require visas that they are unable to meet deadlines. An example of this would be traveling to China, departing in 3–4 days and returning 10 days later, then departing to Russia 3 days after returning from China; this itinerary wouldn’t allow for a person to travel and get visas within the allotted time they have.
The State Department allows the issuance of the 2nd passport when evidence of such a travel conflict is stated and proven. This is achieved through formal requests and flight reservations showing travel intentions.
