Note: You must use this method for any one of the reasons listed below:

• Have never had a US Passport.
• Had a US Passport but I have lost it
• I have a US passport, but it is damaged or excessively worn
• I have a US Passport, but I was under 16 when it was issued

Unless lost, you must submit any previous US Passport you might have in your possession
