Note: You must use this method for any one of the reasons listed below:

• Applicant has a current US Passport but the DATA on the passport has changed or was incorrectly reported on your existing passport. This may be due to name change or gender change or incorrect information as reported on your current passport.

• Your Passport is in Good Condition and not excessively worn.

*You will be given a new full-term passport (10 years); you are not just extending the validity of your existing passport.
